I don't know if there's fic about Kaylee actually becoming the Doctor's companion, though I wouldn't be surprised if it's out there. Have you read Out of Joint by honorh? It's the best Firefly/Doctor Who crossover I've read, and it does feature Kaylee getting to help fix the TARDIS.
